Details: Family Home Health Services is seeking a Physical Therapy Assistant (PTA) to join our growing team in Lansing. The PTA will assist in the implementation of the rehabilitative plan of care under the direction and supervision or a qualified physical therapist.Essential Tasks: Adheres to all Family Home Health Services policies and procedures and maintains standards of practice and documentation that are consistent with State and Medicare regulations and accreditation standards. Provides direct patient care within scope of practice, as deemed necessary by the physician and the physical therapist, in accordance with the established care plan that can be carried out in a safe manner in the patient’s home. Assist the Physical Therapist and Physician in the evaluation of the patient’s current functional status and overall abilities to help determine realistic outcomes. Delivers a variety of Physical Therapy treatments in accordance with the plan of care and accepted standards of treatment specific to patients of various Physical Therapy diagnoses. Provides care based on patient and family needs, identifies goals, interventions and expected outcomes wit patient/caregiver/family involvement. Observes and documents treatments administered and patient reaction to treatments given. Contributes to evaluation and planning process that addresses physical, psychosocial, environmental, and safety factors while considering physician orders, relevant history, patient’s functional status, and availability of willing and able caregiver. Evaluates subjective complaints and objective changes in patient’s status and progress. Evaluates, reports and records patient's status, changes and progress to the Physical Therapist, physician, and the health care team. Observes and documents treatments administered and patient reaction to treatments given. Uses appropriate intervention to meet physical, psychosocial, environmental and safety needs of the patient. Reports changes and reactions to the Physical Therapist, Physician and Home Health team, as necessary. Completes necessary clinical notes legibly and accurately. Documents patient limitations and progress, and submits paperwork in a timely manner. Provides patient/caregiver education with a focus on goal and outcome achievement as established in the plan of care, documenting patient response to teaching in clinical notes. Uses teaching methods as appropriate, including demonstration, verbal instruction and written material. Participates in mandatory staff meetings, in-services and in patient case conferences/coordination of care communication. Assumes responsibility for equipment, cleaning, and maintenance, as appropriate. Fulltime PTA must work a minimum of 60 visits bi-weekly. Performs any other duties within scope of practice and pertinent to patient care as assigned by the Physical Therapist or Director of Nursing. Maintains confidentiality of OASIS data set, patient medical records, all patient protected health information, and Family Home Health Services material and business. Teaches other members of the patient care team, including home health aides, patients and caregivers, as appropriate, regarding rehabilitation program, use of physical therapy devices and therapeutic activities they can perform safely and independently with the patient to more effectively restore safe physical function. Participates in supervised visits with the PT, as indicated, based on patient progress/need. Participates in Performance Improvement activities and identifies areas for improvement.
