Immediate openings for Baltimore County, Baltimore city, Harford County . Home care experience a plus. Guaranteed part time or guaranteed per diem positions available. Competitive salary rates for position. Home visits include physical therapy evaluations and developing HEP. Must be available minimum of 3 days a week for case management of clients.
Minimal OASIS assessments required. Most admission OASIS performed by RN.
