We understand how important it is for patients to remain at home during their recovery.TotalCare Home Health of MA has an immediate Per Diem openings for Experienced Per Diem Physical Therapists. The Physical Therapist is responsible for evaluating, developing a plan of care and coordinating the therapy care of patients in coordination with orders from a physician.We are currently looking for Per Diem PT candidates in the Central MA & North Shore service areas that we cover.Essential Duties and Responsibilities: Performs initial and on-going assessment to determine level functioning, Including OASIS assessment at appropriate time points. Develops and revise the plan of care in consultation with the physician and other care-team members. Provides therapeutic treatments and evaluation of equipment needs as applicable. Prepares timely and appropriate clinical assessment and progress notes in accordance with accepted documentation standards. Evaluates outcome of care.6. Supervises Home Health Aides as appropriate. Advises and consults with the family and other agency personnel, as applicable. Evaluates patient in the home setting by applying muscle, nerve, joint and functional ability test. Identifies patient’s physical therapy needs for home care and develop long and short goals to meet those needs. Applies physical therapy procedures to achieve goals and re-asses the patient periodically, as needed. Submits a weekly written report of each visit within established time-frame. Records and reports the patient’s reactions and conditions to physicians and agency personnel per the interdisciplinary communication policy. Promptly submits to the therapy manager a request for doctor’s orders for initiation of therapy services, renewal of orders and discharge per PPS/OASIS timelines. Answers questions regarding physical therapy related to present and future care. Encourages carry-over in the home by teaching nursing staff, family and/or Home Health Aides the appropriate physical therapy regime. Makes specific recommendations regarding adaptive equipment, special equipment and self-help devices, and instruct both verbally and in writing, the proper use and care of equipment for the patient/family, and nurse, home health aide. Works in interdisciplinary model to integrate physical therapy visits into the overall plan of care. Participates in staff education programs and meeting as requested. Participates in training program of the Home Health Aides and Quality Assurance Programs. Develops patient’s plane of discharge from services. Performs other duties as required.
