Bremerton Health & Rehab Center is a 125 bed skilled nursing facility located in Bremerton, WA not far from the regional hospital, Sheridan Park Senior Center and next door to an assisted living center. Our facility is scheduled to undergo remodeling in the first quarter of 2011. With this remodel we will offer an independent apartment suite to help with transition back to community living. Our rehab gym includes a variety of state of the art equipment including those for Accelerated Care Plus modalities. SUMMARY OF POSITION: To provide physical therapy services in accordance with policies and procedures and in compliance with state and federal regulations. Direction and supervision of physical therapy assistants as indicated. Essential Functions (But not limited to) 1. Perform evaluation and treatment of movement disorders in accordance with physician orders. 2. Treats all residents, visitors, and staff with courtesy. 3. Compliance with federal and state regulations, professional guidelines and code of ethics for the professional of physical therapy and provision of physical therapy services. Essential Qualifications (But not limited to)
1. Graduate of an accredited entry level physical therapy program. 2. State credentialing/licensure as required. 3. Synthesis of knowledge related to anatomy, physiology, and pathology for effective evaluation and treatment of movement disorders. 4. Sound clinical judgment relating to the provision of physical therapy services.
