This includes short-term transitional care, long-term nursing care, Alzheimer's care, assisted living, rehabilitation, and other health related services. Ambassador Rehab Services is uniquely positioned to deliver one of the most effective rehabilitation programs in the long-term health care industry. Our exclusive delivery system has been successfully implemented in more than 60 health care facilities in multiple states.
In addition to providing exceptional Occupational, Physical and Speech Therapy, the Ambassador Rehab Services team also offers valuable support and training to facility partners in the following areas:
• Appropriate RUG Utilization Practices
• Development 7 support of programs for long-term care residents
• Census building Programs
• Continued Education Programs for facilities
• Outpatient Programs
Administrators tell us they “must have" communication and integration within their system and their facility. We have responded by:
• Establishing Rehab, MDS, and Nursing coordination to deliver optimum levels of therapy.
• Attending key facility meetings (Director of Rehab)
• Building communication with Restorative Nursing to ensure Plan of Care is followed.
• Participating in census development with the facility marketing staff to create a cohesive marketing effort.
Job Requirements
We are looking for a full time Physical Therapist for our skilled nursing facility in Floyd, VA! Must be a graduate of an accredited Physical Therapy Program with a current Virginia license, or eligible for licensure.
Skyline Nursing and Rehabilitation Center is a 90-bed nursing facility, which provides skilled and long-term nursing care in addition to respite and hospice care. Skyline is located in the heart of Floyd County in the mountains of Southwest Virginia, within minutes of local physicians and less than an hour from 5 major hospitals. Our Special Care Unit is designed to provide dignified living to those individuals in the first and middle stages of Alzheimer's disease and related disorders. The Special Care Unit includes a spacious living area and enclosed courtyard which encourages freedom of movement while promoting the resident's safety. Our nursing, social service, activity, and therapy professionals are dedicated to enhancing the physical, mental, and psychosocial well being of each resident.
