Program: Home Health
Status: Full-time, Part-time or Per Diem
POSITIONS AVAILABLE IN specific geographic areas: South San Francisco to Redwood City, Redwood City to Sunnyvale, Sunnyvale to San Jose
Description:
The Physical Therapist develops and coordinates the physical therapy plan of care and provides direct patient care to a caseload of homebound patients within a defined geographic area. Coordinates the care provided by the interdisciplinary team of nurses social workers, occupational and speech pathologists and home health aides.

Job Requirements
Licensed as a Physical Therapist in the State of California. Minimum of two years of physical therapy experience. Strong assessment and teaching skills. Well-organized, independent self-starter. California drivers license, automobile and automobile insurance. Current certification in cardiopulmonary resuscitation. Basic computer skills.
