Under the direction of the Facility Director/Administrator, the Physical Therapist provides hands-on direct care including evaluation, treatment, education, updating the patient's treatment, and discharge planning as a member of the treatment team.

Under the direction of the Facility Director/Administrator, the Physical Therapist provides hands-on direct care including evaluation, treatment, education, updating the patient's treatment, and discharge planning as a member of the treatment team.
Essential Functions:
- Performs client tests, measurements and evaluations as appropriate.
- Plans and prepares written treatment programs based on evaluation, age-related and diagnosis-related policies.
- Administers treatments as prescribed and according to the treatment plan.
- Monitors all patients under his/her care; including those being seen by a technician and assures appropriate treatment and documentation.
- Evaluates effectiveness of treatment and adjusts the treatment to achieve maximum benefits.
- If treatment needs to be altered from that prescribed or planned, the therapist receives physician approval for such changes.
- Participates actively in team conferences, team planning processes, and goal setting for each client.
- Maintains timely and appropriate documentation of reports, updated treatment plans, discharges and other client-oriented documentation.
- Provides inservices appropriate to staff.
- Seeks assistance from other staff members or supervisor when there are conflicts (i.e. scheduling, difficult patients or other issues).
- Performs other job-related duties as assigned by the Facility Director/Administrator.
- Maintains the highest level of client rapport.
- Actively involved in the Quality Assurance Program implemented by the Facility Director/Administrator.

Weekends Required: Every Other
Status: Every Other
- Must be a graduate of an accredited program and be a Physical Therapist in good standing.
Experience:
- Some experience in dealing with a broad based rehabilitation facility is preferred.
License/Certification:
- Current Oregon Physical Therapist License.
- Basic Life Support (BLS) certification required within 30 days of hire.
