Franklin Health and Rehab is seeking licensed Physical Therapist for on call PRN services. As part of our team you will:

·         Provide physical therapy service to adult residents according to plan of care

·         May train resident in the use of prosthetic devices

·         Prepare and submit clinical summaries based on attainment of goals

·         Identify resident and family caregiver needs

·         Other duties as assigned by Rehabilitation Manager

Qualifications:

Be a graduate of accredited APTA Program with a BS or MA in Physical Therapy. Licensed to practice in state of VA. Be driven and excited about working within a team environment.

PRN opportunity for Physical Therapist at this 120-bed facility. Evaluates residents, develops effective treatment plans, and obtains approval for service from the referring physician. Treats residents according to the physician's treatment plan. Participates in discharge planning. Records evaluation, treatment, and charges. Supervises Physical Therapy Assistants and Aides. Screens residents for potential benefit from PT intervention.
