Description Summary:

 

• The Physical Therapist provides rehabilitative care for patients needing assistance and training in ambulation, transferring, gait training and home exercise programs.

 

 

• The PT works as part of the interdisciplinary team including RN , LPN , PT , PTA , OT , COTA , SLP , MSW and Home Health Aides.

 

 

• The PT reports to the Rehab Director or Clinical Coordinator.

What is Home Health?

 

• Nursing and Therapy team members visit patients in their homes on an intermittent basis according to doctor’s orders.

 

 

• Our skilled professionals work one on one with patients, caregivers and family members.

 

 

• Home Health allows health care professionals to utilize all of their accumulated knowledge and skills to help individual patients reach their goals as part of a multi-disciplinary care team.

 

 

• Staff perform 5-7 patient visits daily on average.

 

 

Education and Experience Summary:

 

• Graduate from a physical therapy curriculum approved by either the APTA, Committee on Allied Health Education and Accreditation of the AMA, or Council on Medical Education of the AMA and APTA

 

 

• Minimum of 1 year experience as a PT

 

 

• Home Health experience preferred

 

 

• Able to evaluate patient needs through ongoing assessment and revise care plans based on changes in patient condition

 

 

• Must have CPR certification

 

 

• Valid driver’s license and auto liability insurance with reliable transportation to travel to patient homes per patient schedule and agency needs.
