Physical Therapist
This is a great opportunity to deliver high quality, uninterrupted patient therapy in the comfort of the patient's home, while providing educational support to patient and family.
Home Health staff enjoy the flexibility of maintaining their own schedules.
Responsible for evaluation and treatment of disabilities, injuries, and diseases in accordance with the orders of the referring physician.
Performs assessments and establishes a plan of care.
Maintains documentation (laptop computer provided) for each patient in compliance with Agency policies and HCFA regulations.
Serves home bound patients primarily in the counties of Leavenworth and Wyandotte counties in Kansas and occasionally nearby northern Missouri communities.

Job Requirements
Proven interpersonal and organizational skills, focused on quality care and patient education, computer knowledge, and positive attitude and energy to share with our patients and staff.
KS licensed Physical Therapist (dual licensed in MO helpful)
valid driver license and proof of auto insurance
