Overall Responsibility:  The Medical Social Worker provides evaluation and treatment to patients having problems of social and personal adjustment that interfere with independent living or are a reaction to the medical diagnosis.  Medical Social Worker is responsible for developing a treatment plan and providing individual, family or group therapy.  Initial evaluation shall include patient and/or family goals.  Ability to utilize community services and agency support are necessary.

Duties Include:

• Provides psychosocial evaluation, diagnosis, treatment, and referral for patients and/or families.  Evaluation shall include patient and/or family goals.
• Interprets results of evaluation and recommends treatment as indicated to the patient/family referral source.
• Provides services that include but are not restricted to community referral, individual therapy, family therapy, group therapy, consultation, or education.
• Aids the health care team in identifying, understanding and integrating the significance of psychosocial factors in relation to the patient’s illness, treatment, and recovery.
• Plans and leads individual and family therapy, patient and family group and leads family conferences.
• Provides a written report for patient medical record (evaluation, progress notes, and discharge summary) with copies to referral source, patient, and family when indicated.
• Maintains accurate, up-to-date records in the client’ chart.

Education:

 Master’s degree in Social Work

Experience:

Medical Social Work experience preferred

Additional Requirements:

·         Physical within 6 months prior to first patient contact

·         Current TB results (within last 12 months)

·         Current CPR Certification

·         Must have valid driver’s license
