Job Description
Responsible for providing comprehensive diagnostic, consultative, and/or treatment services for patients with disorders that cause impaired functioning in activities of daily living. Conducts therapy modalities designed to improve patient quality of life. Under the direction of the individual physician's plan of care and supervision of a physical therapist, the COTA will perform home occupational therapy modalities and treatments, with accompanying clinical documentation and patient follow-up.
Job Requirements
Education: Meets the requirements for certification as an occupational therapy assistant established by the American Occupational Therapy Association; or has 2 years of appropriate experience as an occupational therapy assistant, and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service, except that such determinations of proficiency do not apply with respect to persons initially licensed by a state or seeking initial qualification as an occupational therapy assistant after December 31. 1977 (Medicare Conditions of Participation for Hospice Agencies). Experience: At least one year clinical experience, preferably in Hospice care. Licenses/Certificates:Licensed to practice occupational therapy in the state of practice. Possess a current CPR card. Reliable transportation, current driver's license, and proof of auto insurance.
